Squint Management

Squint Management: Expert Correction for Eye Alignment at Dr. Mahfuj Eye & Oculoplasty Centre

At Dr. Mahfuj Eye & Oculoplasty Centre, we understand that a squint (strabismus) can significantly impact both vision and confidence, especially in children. Led by Dr. Md. Mahfujullah, a highly skilled Eye Specialist, Phaco & Oculoplastic Surgeon, our centre provides comprehensive diagnosis and advanced management for squint, helping to correct eye alignment, restore binocular vision, and improve overall eye health for patients of all ages.

What is Squint (Strabismus)?

Squint, medically known as strabismus, is a condition where the eyes do not align properly. Instead of looking in the same direction at the same time, one eye turns inward, outward, upward, or downward while the other eye focuses normally. This misalignment can be constant or intermittent, and it may affect one eye or alternate between both eyes.

When the eyes are misaligned, they send different images to the brain. In adults, this can lead to double vision. In children, whose brains are still developing, the brain may learn to ignore the image from the misaligned eye to avoid double vision. If left uncorrected, this can result in permanent vision loss in the weaker eye, a condition called amblyopia (lazy eye).

Types of Squint

Squint is classified by the direction of the misaligned eye:

  • Esotropia: An eye turns inward (crossed eyes).
  • Exotropia: An eye turns outward (wall eyes).
  • Hypertropia: An eye turns upward.
  • Hypotropia: An eye turns downward.

Squint can also be categorized as:

  • Constant vs. Intermittent: Always present or appearing sometimes.
  • Unilateral vs. Alternating: Affecting only one eye or switching between both.
  • Accommodative: Caused by uncorrected farsightedness.

Symptoms of Squint

Recognizing the signs of squint is crucial, particularly in children. Symptoms can include:

  • Visible Misalignment of the Eyes: The most obvious sign, one eye not looking straight.
  • Frequent Rubbing of Eyes: Or blinking/squinting in bright light.
  • Head Tilting or Turning: To try and align the eyes or see better.
  • Double Vision (Diplopia): More common in adults with recent onset squint.
  • Depth Perception Problems: Difficulty judging distances.
  • Eye Strain or Headaches: Especially after reading or close work.

Causes of Squint

Squint can be caused by various factors, often involving problems with the eye muscles, the nerves that control them, or the brain’s ability to coordinate eye movements:

  • Genetic Factors: A family history of squint or amblyopia increases risk.
  • Refractive Errors: Significant uncorrected farsightedness (hyperopia) can lead to accommodative esotropia as the eyes over-converge to focus.
  • Nerve Problems: Issues with the cranial nerves that control eye muscles.
  • Brain Conditions: Cerebral palsy, hydrocephalus, or stroke can affect eye coordination.
  • Injuries or Trauma: To the eye muscles or nerves.
  • Systemic Illnesses: Certain diseases like thyroid eye disease can affect eye muscles.
  • Cataracts or Other Vision Impairments: Any condition that severely reduces vision in one eye can lead to that eye drifting.

The Importance of Early Diagnosis and Treatment

For children, early diagnosis and treatment of squint are paramount. The visual system develops rapidly in early childhood. If a squint is left untreated, the brain may permanently suppress the image from the misaligned eye, leading to irreversible amblyopia (lazy eye). Prompt intervention can often prevent or significantly improve this vision loss, allowing for proper development of binocular vision (the ability of both eyes to work together to create a single, three-dimensional image).

Diagnosis of Squint

Diagnosing squint involves a comprehensive eye examination by an experienced ophthalmologist. Dr. Mahfujullah will:

  • Detailed History: Including family history, onset of symptoms, and any other medical conditions.
  • Vision Acuity Test: To assess the vision in each eye individually and identify amblyopia.
  • Refraction: To determine the need for glasses.
  • Eye Alignment Tests: Various tests, such as cover/uncover test and prism cover test, to accurately measure the degree and type of eye misalignment.
  • Ocular Motility Assessment: To evaluate the movement of each eye.
  • Fundus Examination: To check the health of the retina and optic nerve.

Squint Management and Treatment Options

The goal of squint management is to correct the eye alignment, improve vision, and restore binocular function. Treatment plans are highly individualized, depending on the type, cause, severity, and age of the patient. Dr. Mahfujullah offers a range of effective treatment options:

  • Optical Correction (Glasses):
    • For accommodative esotropia, glasses to correct farsightedness are often the first line of treatment and can completely resolve the squint.
  • Patching (Occlusion Therapy):
    • For amblyopia, patching the stronger eye forces the weaker, misaligned eye to work harder, improving its vision. This is particularly effective in children.
  • Vision Therapy (Orthoptics):
    • Eye exercises to improve eye coordination and focusing skills, often used for specific types of squint or to supplement other treatments.
  • BOTOX Injections:
    • In select cases, particularly for certain types of adult strabismus or as a diagnostic tool, Botulinum Toxin (BOTOX) can be injected into specific eye muscles to temporarily relax them and improve alignment.
  • Squint Surgery (Strabismus Surgery):
    • Often the definitive treatment for many types of squint. It involves surgically adjusting the length or position of the eye muscles to realign the eyes.
    • The surgery is typically an outpatient procedure.
    • It aims to achieve proper eye alignment, improve cosmetic appearance, and facilitate the development or restoration of binocular vision.
    • Multiple surgeries may sometimes be required for optimal alignment.

Dr. Mahfujullah will thoroughly discuss the most appropriate treatment strategy, explaining the benefits, risks, and expected outcomes specific to your condition.

Benefits of Squint Treatment

Correcting squint offers significant advantages for patients:

  • Improved Vision: Prevents or treats amblyopia (lazy eye), especially in children.
  • Restored Binocular Vision: Enables both eyes to work together, leading to better depth perception.
  • Enhanced Appearance and Confidence: Correcting eye misalignment can greatly improve self-esteem and social interaction.
  • Reduced Symptoms: Alleviates double vision, eye strain, and headaches.
  • Better Quality of Life: Allows for participation in activities that require good depth perception, like sports.
